
Case Manager

When someone has sought emergency medical care due to a mental health crisis, the hospital medical team may refer them to Crisis Diversion. This program helps individuals with: assistance navigating and accessing care systems and resource, development of comprehensive treatment and care plans, developing and repairing support systems and intensive case management. As an intensive case management program, it is designed to reduce unnecessary psychiatric hospitalizations by providing stabilization and linkage in the community.
Position Details
Position Title: Case Manager
Position Type: Full Time; 40 Hours/Weekly

Department: Crisis Diversion
Responsibilities
Provide in‑vivo supportive counseling to consumers and those involved in the consumer's network of care
Link consumers to on‑going behavioral health services and other related community supports/services as appropriate
Provide on‑going assessment of client's well being, clinical symptomology, behavioral changes and medication compliance
Assist consumer's with ADL's through modeling and direct assistance
Provide crisis intervention services as needed
Other related duties as assigned
Qualifications
Bachelor's Degree in a Human Services field.
