Responsibilities
Conduct OT evaluations and develop individualized treatment plans for acute care patients, focusing on ADLs, mobility, cognitive function, and discharge planning.
Implement therapy interventions to improve functional independence; monitor progress and adjust plans as needed.
Document evaluations, treatment progress, and discharge recommendations in the electronic health record per policy and regulatory standards.
Collaborate with an interprofessional team (physicians, nurses, therapists) to coordinate patient care and discharge planning.
Educate patients and families on home safety, adaptive equipment, and care strategies; provide discharge planning guidance as appropriate.
Maintain patient safety, infection control, and patient privacy while delivering evidence‑based OT care.
Requirements
Active OT license/registration per state regulations
Skilled Nursing experience preferred
Ability to deliver OT services in an acute care setting
Willingness to work a rotating schedule with alternate weekend rotation
Proficiency with documentation in electronic health records and interdisciplinary communication
Strong clinical judgment and patient‑centered approach
