Position Overview
We are seeking a Licensed Professional Counselor (LPC) to provide psychotherapy and psychosocial support to residents in skilled nursing facilities. The LPC will conduct assessments, deliver individual and group therapy, collaborate with the interdisciplinary team (IDT) and ensure regulatory compliance.
Key Responsibilities
Conduct initial diagnostic assessments and develop individualized treatment plans.
Provide psychotherapy services tailored to resident needs.
Deliver individual, group, and family counseling focused on depression, anxiety, grief, adjustment, and behavioral health issues.
Document sessions in compliance with Medicare and facility requirements.
Participate in care plan meetings and interdisciplinary team discussions.
Support residents and families in coping with illness, placement, and end-of-life issues.
Collaborate with Strive psychiatric nurse practitioners and facility staff to ensure coordinated care.
Educate facility staff on recognizing psychosocial needs and appropriate referral processes.
Qualifications
Master’s in counseling or related field (regionally accredited; CACREP preferred but not required)
Enrolled or eligible for enrollment as a Medicare Part B provider.
Experience working with geriatric populations or in long-term care preferred.
Strong interpersonal, documentation, and communication skills.
Ability to work independently within a collaborative, team-based environment.
