How you’ll make an impact in this role
Provide coordination of patient care throughout the healthcare continuum.
Provide coordination of communications with patients, families, physicians, and healthcare team members.
Coordinate patient scheduling with appropriate resources or clinical services and maintains required documentation.
Support standard patient access initiatives across the healthcare continuum.
Provide support with navigating the healthcare system and assists patients with utilizing available community support services.
What minimum requirements you’ll need
Education:
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
What additional preferences we're seeking
Rotating On-Call Schedule: Sat and Sun 8am-5pm - in rotation with staff
