How you’ll make an impact in this role
Assess complex emotional and social barriers to develop personalized care plans that address each patient’s unique needs and preferences.
Coordinate with interdisciplinary teams to streamline the transition from admission to discharge, ensuring a safe and continuous journey of care.
Advocate for patient rights and preferences within the medical system to ensure the care provided is respectful and person‑centered.
Empower patients and families through education on financial and community resources, helping them build the stability needed to manage their diagnosis.
What Minimum Requirements You’ll Need
Licensure / Certification / Registration:
Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.
Certified Social Worker preferred.
Education
Bachelor’s degree in Social Work required.
Work Experience
3 years of experience preferred.
What Additional Preferences We’re Seeking
Must have Social Worker license.
Masters degree in Social Work required.
