Program Counselor (Case Manager)
We are seeking a Program Counselor (Case Manager) to join the Vista team to coordinate, monitor, and document all case management services for members in our Engage Program.
Responsibilities
Develop written program plans for each member that evaluate progress, determine individual goals, and identify programmatic service needs.
Implement systems supporting each assigned student’s programmatic and individual goals across four main areas: social/emotional, vocational, life skills, and community involvement (e.g., recreation and transportation).
Provide individual and family counseling for members.
Communicate member needs, issues and goals to all staff members and other involved parties.
Maintain all necessary records and documentation, including monitoring students’ medical, wellness and financial needs.
Requirements
Bachelor’s Degree or one (1) year of related experience. Vista values work experience and may waive the Bachelor’s requirement for a strong work background in a related field.
Valid driver’s license with a safe driving record.
Location
This position is based at our Westbrook, CT living community and will also support members who reside between Guilford and Old Saybrook.
Job Type/Shift Information
Full-Time, Monday‑Friday 8am‑4pm or 9am‑5pm.
Salary
$51,000, plus eligible mileage reimbursement.
