Coor-Population Health - VBC Operations - Full Time
The Population Health Coordinator is a member of the patient-centered care team responsible for collaborating with integrated case managers to provide support to patients and their families that optimize health outcomes. Interfaces with insurance companies, community agencies, clinical entities and specialized programs to support accurate communication and operational transactions.
Qualifications - External
EDUCATION/EXPERIENCE REQUIRED:
Associate Degree from an accredited academic institution required.
One-year experience working in a health care or community setting required.
Ability to assess, identify barriers and work in collaboration with case managers and other members of the care team to support patient's social and clinical needs.
Strong customer service and interpersonal skills with the ability to interact with all members of the health care community.
Strong sense of compassion with the ability to successfully advocate for patients and their families.
Proficiency with Microsoft Word, Excel and PowerPoint.
Ability and willingness to self-motivate, self-manage and maintain flexibility in an ever changing environment.
