Summary
Facilitates, coordinates, and manages an assigned caseload to promote evidence based clinical focused outcomes. Manages utilization management supporting medical necessity, determine appropriate level of care, and resource utilization, and evaluates the need for services for continuity of care and denial prevention. Assess, evaluate, and coordinate for safe and appropriate discharge planning needs in compliance with state and federal regulatory requirements, TJC accreditation standards, and policies.

Associate degree
In a Nursing Program from an Accredited School of Nursing
Experience
Two Years of acute care experience required
