The Neuromedical Center Rehabilitation Hospital is in need of a PRN Licensed Clinical Social Worker.
Responsibilities
Complete discharge planning and provide counseling for neurologically complex inpatients. Attend weekly team meetings to address patients needs and provide resources to patients and families. Responsibilities also include case management.
Qualifications
Must be a Licensed Clinical Social Worker (LCSW)
One year or more experience
Excellent customer/patient service and communication skills
Masters degree in social work from an accredited university
Experience in Case Management
