Case Manager – Children, Youth, and Family Services (CYF)
The Metro Boston Area Child, Youth, and Family Services Team operated by the Department of Mental Health (DMH) seeks a dynamic candidate to provide Case Management services for eligible youth up to 22 years old, with majority under 18.

Duties and Responsibilities

Provide case management services for eligible youth up to age 22.

Engage families/guardians in the process to align with CYF Family Collaboration Principles.

Develop creative engagement strategies and maintain regular face‑to‑face contact with clients, families and community providers to support goal attainment and community tenure.

Collaborate with internal and external providers and families to ensure integrated planning and service delivery.

Prepare and maintain documentation per DMH regulations and EHR business rules, including progress notes and required documentation.

Conduct home visits and monitor safety of the environment.

Coordinate and facilitate multi‑service and system team interactions, including scheduling and chairing care planning meetings to discuss service plan effectiveness.

Required Qualifications

Knowledge of psychosocial problems, types and symptoms of mental and emotional disorders.

Knowledge of counseling, interviewing, crisis intervention, motivation, and reinforcement techniques.

Knowledge of programs and services available to youth, young adults, and their families in MA.

Ability to understand, explain and apply laws, rules, regulations, policies, procedures, specifications, standards and guidelines governing agency activity.

Capacity to make case assessments by examining records and observing clients.

Excellent oral and written communication skills, with ability to give clear instructions.

Ability to make decisions, act quickly and maintain calmness in stressful or emergency situations.

Preferred Qualifications

Master’s degree in human services, psychology, social work, or related field.

3+ years of experience in child/adolescent behavioral health services.

Experience working with youth and families in community and home‑based settings.

Special Requirements
Travel is required. This position manages cases in the DMH Metro Boston catchment area and may involve occasional statewide travel. Current and valid Massachusetts Class D (or equivalent) driver’s license is required.

Salary
68,435.12 – 93,470.00 Yearly
