Driver/Transportation Care Assistant

Sunrise PACE Center Region VII Area Agency on Aging/Sunrise PACE is accepting applications for a highly motivated Driver/Transportation Care Assistant. We are a growing non-profit organization based in Bay City, MI. The PACE center and job location is located in Fort Gratiot (St. Clair County).
The PACE Driver/Transportation Care Assistant is responsible for providing safe transportation to participants in a timely, competent manner, and communicating important information relevant to participant care or the overall care plan to the Interdisciplinary Team. He/she is critical to setting the tone and desired experience expected by PACE for each participant on trips to the Center and other appointments. The Driver's knowledge and support of the mission and values of PACE are important to successful performance, as are commitment to and demonstration of a spirit of teamwork.
Requirements for this position:
Ability to clearly communicate orally and in writing.
Interest in working with medically frail elders.
Valid Chauffeur's Driver's license.
DOT Physical.
Driving record free from major infractions, including but not limited to, DUI, negligent, reckless, or excessive minor infractions and convictions.
Demonstrated ability to safely operate a wheelchair-accessible van or vehicle.
Current CPR and First Aid training or completed prior to working independently after hire.
Education: High School diploma or equivalent required.
Credentials/Licensure Required: Valid Chauffeur's Driver's license with a good driving record.
Experience working with a frail or elderly population is strongly preferred.
Experience with medical transportation preferred.
Must be able to read, write, and speak the English language and be understood.
Must be able to handle information in a confidential manner at all times.
Must be able to deal professionally and diplomatically with people, both in person and on the telephone.
Must have good interpersonal skills.
Must be able to perform wheelchair and ambulatory transfers between home and van, and the Center and van.

Physical demands: Employee must be able to sit for extended periods of time. Employee will also be actively present throughout the PACE center, engaged with employees, participants and with staff. Employee is required to use hands and fingers, especially for typing on the computer and using the mouse. Employee must be able to talk and hear, particularly for daily interactions with staff and elderly participants.
Work Environment: Employee must be able to work in a busy and demanding clinic environment with moderate to loud noise levels.
