CNA/Personal Care Attendant (Home Health)
Region VII Area Agency on Aging/Sunrise PACE is accepting applications for a highly motivated CNA/Personal Care Attendant (Home Health Care) for Sunrise PACE. We are a growing non-profit organization and our Sunrise PACE center is in Fort Gratiot (St. Clair County). Looking to fill a 10 hour afternoon shift with weekend rotation (every 3rd weekend).
Purpose of the position: Under the direction and supervision of the PACE Center Manager, the CNA/Personal Care Attendant provides home and personal care services to Sunrise PACE participants in their in home setting primarily. Provides physical assistance and attendant care to enrollees across care settings (home, center and/or transportation). Provides assistance with Instrumental Activities of Daily Living (IADLs). Provides assistance with nursing care, restorative therapies, activities and meals across care settings, but primarily in the home. Demonstrates the knowledge and skills necessary to participate in assessing, planning care for and providing service to frail elder enrollees according to assigned responsibilities and PACE Program standards.
Requirements for this position:
Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant care
Pass a criminal background check which will verify that the candidate is: not excluded from participant in Medicare & Medicaid programs
High school diploma or equivalent
CNA certification required
One year experience working with the frail or elderly population or, in the absence of such experience, to receive appropriate training from the Sunrise PACE on working w/frail or elderly population upon hire
Must pass and maintain annual competencies for role
Ability to perform housekeeping duties
Possess discretion, tact, patience and good judgement. Empathetic and compassionate attitude in caring for the elderly
Ability to understand and carry out non-technical verbal and written directions, record messages and keep records.
Ability to get along well with others and gain the cooperation of plan participants
Able to handle difficult situations in a professional manner
Routine work involving the personal care and program activities of participants.
Proven record of excellent attendance
Ability to maintain flexibility in schedule and respond to unexpected emergencies and changes in workload
Ability to spend long periods of time on feet and do heavy lifting on occasion
Licensed driver with reliable transportation
