Job Title

This position is only open to current Colorado residents. Bruce McCandless Community Living Center Colorado State Veterans Homes are State-owned and operated skilled-nursing facilities dedicated to serving those who have served us.
Job Description

Summary of Position: This position will help with general resident activities, the one-to-one program, sensory stimulation program and outings, and work in the Memory Care environment as well. This position will help implement the Eden Alternative style of nursing home management as it applies to daily activities of residents. It also facilitates the participation of all facility staff in culture change activities.
Primary Duties:
Plans activities that expands and enhances the lifestyle of each resident in the five categories required by the State and Federal regulations of social/recreational, intellectual, physical, spiritual and creative.
Interview residents and family upon admission to determine personal preferences, activity interests and leisure requests.
Responsible for offering a variety of activity choices, maintaining scheduled activities as planned, and facilitating spontaneous activities that meet social, physical, spiritual, creative and intellectual needs of all residents.
Responsible for preparing in advance of the scheduled event, setting up necessary supplies, communicating to residents the activity is about to begin, assisting residents to/from the activity, providing encouragement to residents throughout the activity, and maintaining a positive atmosphere that enhances participation. Seeks guidance for difficult residents, supplies that are not readily available, and any other special needs. Provides activities of daily living (ADL) care during activities or as needed including but not limited to transferring residents, assistance with feeding, personal care, toileting, grooming, changing clothing, answering call-lights, and other duties as needed.
Responsible for: maintaining daily documentation for the Activity Department through participation records, progress notes and behavior charting.

Responsible for conducting quarterly or upon significant change activity assessments of all existing residents on-going. Responsible for developing appropriate activity plan of care and communicating with the resident, family and interdisciplinary team weekly during care plan meetings.
Responsible for driving various facility vehicles for scheduled outings such as luncheons, fishing trips, tours, etc., and providing back-up transportation for medical purposes including hospital transfers or appointments.

Special Qualifications:
Must provide proof of Certified Nursing Assistant class completion AND must obtain C.N.A. license within 4 months of graduation
Conditions of Employment: Full background check and reference check required. This position requires State of Colorado residency at the time of application (unless otherwise identified in the posting), and residency within the state throughout the duration of employment in this position. Motor Vehicle Check, if the employee will drive a state-owned vehicle, either in an on-going capacity while performing their normal day-to-day job duties OR on occasion for training/meetings/client visits/etc. Must possess a valid, non-restricted Colorado Driver's License or an non-restricted U.S. Driver's License for positions within 30 miles of the CO state border. Out-of-State Driver's License holders must obtain a valid, non-restricted Colorado Driver's License within 30 days of employment start date. Other Conditions of Employment specific to Program. Former State employees who were disciplinarily terminated or resigned in lieu of termination must:
