RN - Hospice

Hospice Registered Nurse (RN) - Moab, Utah

This is a 13-week contract position.

Days. On-call shifts 1-3 times per week, including 3 weekends during the contract.

We are seeking a dedicated and compassionate Hospice Registered Nurse (RN) to provide skilled nursing care and support to terminally ill patients and their families in accordance with the hospice philosophy. The RN will be an integral part of an interdisciplinary team committed to delivering high-quality end-of-life care.

Responsibilities:

• Perform comprehensive assessments of patients' needs and coordinate individualized care plans.
• Provide pain and symptom management consistent with the hospice plan of care.
• Educate and support patients and families regarding disease processes and end-of-life issues.
• Collaborate with the interdisciplinary team including physicians, social workers, and chaplains.
• Maintain accurate and timely documentation in compliance with relevant policies.

Required Experience / Certifications / Licensure:

• Current Registered Nurse (RN) license in the state of Utah or compact state RN license.
• Minimum of 3 years of nursing experience, preferably in hospice, home health, or palliative care.
• Current BLS Certification.
• Valid driver's license and reliable transportation for home visits.
