Clinical Care Coordinator

This position involves coordinating care on a medical/telemetry unit. The department focuses on acute and swing bed patients. Common diagnoses include pneumonia, rehab, COPD, CHF, pancreatitis, and GI bleed. Nurse-to-patient ratios are 1:3-6. There is one CNA per shift responsible for personal care, vital signs, and weights. RNs will set up and operate respiratory equipment, including BiPAP, CPAP, and high flow oxygen. Tele nurses are required to read their own telemetry strips. The shift schedule may include rotating schedules. Weekends and holidays will be required every other time. Floating to acute/obs/swing bed units may be necessary. RNs may participate in code team responsibilities. Teamwork and a positive culture are emphasized in this role. Successful candidates should be dependable, caring, knowledgeable, and flexible.
