Overview
Job Description Responsible for providing short-term psychotherapeutic services including individual, marital, family and group treatment.
Principal Accountabilities
Develops written individual recovery plans.
Engages to enhance functioning, effective coping, illness management, self-management, wellness and quality of life.
Partners with patient's family, peers, para-professionals and professionals in supporting, monitoring and stabilizing consumers in the community.
Participates in community events, human service meetings as appropriate or mandated.
Provides case management services.
Develops and maintains rich collaborative working relationships with hospital emergency departments, service providers, and agencies.
Maintains timely and complete chart documentation.
Incorporates use of criteria sets in context of utilization management.
Performs other related duties as assigned.
Qualifications
Master\'s degree in social work, Psychology or Counseling.
LSW Required. LCSW or LPC license highly preferred.
