Job Summary
We’re looking for a skilled and compassionate LPN/LVN to provide in‑home nursing care under the supervision of a Registered Nurse (RN). You’ll support patients with daily care needs, monitor their condition, and help ensure safe, high‑quality home health services.
Responsibilities
Provide nursing care according to the RN‑developed Plan of Care.
Monitor and document changes in patient condition; report to RN/physician as needed.
Assist with activities of daily living to promote patient independence.
Educate patients and caregivers within LPN/LVN scope of practice.
Maintain accurate EMR documentation.
Participate in case conferences and coordinate care with the home health team.
Follow infection‑control, HIPAA, and company policies.
Current LPN/LVN license in the state of practice.
Graduate of an accredited practical/vocational nursing program.
Minimum
1 year acute care experience
preferred.
Current CPR/BLS certification.
Valid driver’s license, reliable insured vehicle.
Strong communication, clinical judgment, and ability to work independently.
Qualifications
Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent.
Licensed or registered to practice as a PT in the state of practice.
Preferably at least two (2) years of experience as a PT.
Possesses and maintains good physical stamina and mental health; presents a pre‑employment physician’s health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute.
Has the ability to read and follow written instructions and document care given.
Is self‑directing and has the ability to work with little direct supervision.
Possesses and maintains current CPR certification.
Is flexible and cooperative in fulfilling the role obligations.
Demonstrates tact, patience, and good personal hygiene.
Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
Must be able to verbally communicate in the language of the client and document all client‑related activity practices.
