Responsibilities
Assist the licensed practitioner in identifying patient needs, priorities of care, and goals based on patient factors such as age, background, and beliefs.
Gather, observe, record, and communicate patient responses to nursing interventions; modify the plan of care in collaboration with the licensed practitioner.
Demonstrate competency in department‑specific functions and contribute to the evaluation and adjustment of the care plan.
Function as a member of the multidisciplinary team to implement an integrated, patient‑centered care plan; delegate nursing interventions to others with appropriate skills as needed.
Identify early warning signs of change in a patient’s condition; implement nursing interventions and prescribed regimens in a timely and safe manner, with accurate documentation.
Communicate progress and education needs to patients/families and other team members; assist in discharge planning and provide appropriate education and counseling.
Requirements
Active LPN license in Minnesota (current and in good standing)
Ability to perform nursing assessments, care planning, and documentation within licensure requirements
Comfortable working in an inpatient care setting and collaborating with a multidisciplinary team
Availability for a 13‑week travel assignment with 36 guaranteed hours per week
Strong communication and teamwork skills; ability to coordinate with patients, families, and staff
