Responsibilities
Administer skilled nursing care to patients per physician orders and the established plan of care, under RN supervision.
Coordinate care with the interdisciplinary team and communicate with patients and families; document outcomes accurately.
Participate in planning and implementing patient care with the RN to ensure quality, cost-effective results.
Provide patient/family education on self-care techniques and assess responses to care.
Maintain infection control practices and OSHA standards to protect staff and patients.
Assist the RN/physician with duties related to end-of-life care for Hospice patients; assist with admissions, discharges, and transfers.
May participate in on-call duties as required.
Maintain professional boundaries and promptly report unsafe conditions; engage in annual in-service and performance improvement activities.
Requirements
Current Licensed Practical Nurse license in the state of Colorado.
Current CPR certification.
Hepatitis consent/declination and TB vaccination record on file.
Valid driver’s license and auto liability insurance.
Minimum two years of LPN/LVN clinical experience; home health or hospice experience preferred.
Graduation from an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Proficiency with electronic medical charting and time-keeping software (Homecare Homebase).
Must read, write, and speak fluent English; good attendance required.
