Job Summary

Responsible for administering and directing appropriate nursing interventions as prescribed by a qualified physician or mid-level practitioner. Responsible for effective communication and accurate documentation. Displays professional behavior in accordance with Nursing Code of Ethics and CHS philosophy of service.
Key Responsibilities

Performing the initial nursing assessment, communicating findings with the assigned physician or mid-level
Assessing the wound stage and measurements, documenting care plans, evaluating the patient status in regards to desired outcomes
Identifying patient care situations that require intervention
Participates in quality improvement initiatives, providing education to the patient and/or caregiver, and serving as patient advocate in the delivery and coordination of care
Collaborates as needed with other healthcare providers, Wound Care Center Providers, the Program Director and Medical Director regarding patient needs
Performs other duties as required
