Job Summary
Responsible for administering and directing appropriate nursing interventions as prescribed by a qualified physician or mid-level practitioner. Responsible for effective communication and accurate documentation. Displays professional behavior in accordance with Nursing Code of Ethics and CHS philosophy of service.
Hours of Position
Full‑time (40 hours/week), Days
Key Responsibilities
Perform initial nursing assessment and communicate findings with the assigned physician or mid‑level practitioner.
Assess wound stage and measurements, document care plans, and evaluate patient status with regard to desired outcomes.
Identify patient care situations that require intervention.
Participate in quality improvement initiatives, provide education to the patient and/or caregiver, and serve as patient advocate in the delivery and coordination of care.
Collaborate with other health‑care providers, Wound Care Center Providers, the Program Director and Medical Director regarding patient needs.
Perform other duties as required.
