The Agency has embraced the concept of a fully integrated system of care for all consumers of service. This requires that staff assess the client’s needs holistically and see it as their responsibility to connect the client with services needed both internal and external to the Agency, regardless of the door through which the consumer enters the system.
Under the direction of a designated Clinical Supervisor, the Fee for Service Licensed Social Worker is responsible for direct service to clients in the Agency’s outpatient mental health programs. All services are to be delivered in a professional and ethical manner, consistent with best practice tenets. The Fee for Service Licensed Social Worker will function as part of a treatment team, maintain mandated levels of service, participate in Agency meetings as required and fully comply with Agency policies and procedures.
The Fee for Service Licensed Social Worker will perform initial interview, crisis intervention, and ongoing therapy with individuals, groups and families. The Fee for Service Licensed Social Worker will adhere to all standards of contract compliance, audit readiness, Agency policies and procedures, and all other regulatory requirements. The Fee for Service Licensed Social Worker will work as a member of a team to help ensure the overall integrity and welfare of the Agency.
Meet Agency requirements for qualitative and quantitative documentation: writing progress notes, treatment plans, intake assessments, correspondence regarding clients, termination summaries, encounter forms and any other required documentation.
Ensure referrals are made to appropriate internal and external services as required to meet consumer and family needs.
Respond to all referrals in a timely and professional manner and follow‑up on referrals, recognizing the need for effective collaboration to insure the highest levels of service provision.
Provide direct service: conduct individual, family, marital and group therapy as dictated by the client’s clinical needs.
Caseload size dependent on agreed schedule and hours worked, not exceeding 20 hours a week.
Monitor and coordinate psychiatrist psychopharmacology appointments.
Coordinate physical, medical, and psychological needs for clients.
Complete and submit all documentation required for Agency reimbursement within 24 hours of session.
Maintain all applicable standards of audit readiness.
Attend and actively participate in supervision.
Participate, as needed, in Agency response to serious incidents.
Participate in meetings with supervisor and other staff for trainings, planning and evaluation of individual work and agency functioning.
Supervision with a consultant LCSW toward clinical licensure; agency pays an external contractor.
Up to 21 hours of CEU training (training paid by Agency; fee‑for‑service staff do not receive pay for training time).
Reimbursement for Services
Brief session (30 minutes): $25 per session
Full session (45 minutes): $45 per session
Psychosocial plus SBIRT (~1 hour and write‑up): $60 per session
Language differential: $5.00 per session
Agency required training: $25 per hour
Fee‑for‑service clinician payment is based on the delivery of a pre‑approved face‑to‑face service.
Documentation must be received and certified by supervisor prior to payment.
The agency is not responsible for reimbursing if the service scheduled does not take place.
Any ancillary services such as revision of treatment plans, completion of documentation, and submission of appropriate billing are an integral part of service provision and will not be reimbursed beyond the session and/or evaluation rate.
Qualifications
Master’s Degree in Social Work from a nationally accredited institution.
Valid NYS LMSW required.
Obtain NPI immediately upon employment.
Maintain all licensure/certifications for which employee is educationally and professionally eligible in NY State.
Excellent time management, organizational and computer skills, ability to evaluate and intervene in crisis situations, flexibility, and excellent oral & written communication skills.
Bilingual Spanish or Creole speaking preferred but not required.
