Promise Point Care is seeking a compassionate and dedicated Case Worker to support and empower youth in a structured, trauma- informed environment. The Case Worker will play a vital role in helping youth develop life skills, achieve personal goals, and successfully transition into independent living.
Key Responsibilities
Manage a caseload of youth and provide ongoing support services
Develop and implement individualized service/treatment plans
Conduct assessments and monitor client progress regularly
Advocate for youth needs and coordinate with external agencies (schools, counselors, courts, etc.)
Maintain accurate and up-to-date case documentation and reports
Provide guidance, mentorship, and crisis intervention when needed
Facilitate life skills development (education, employment readiness, coping skills)
Qualifications
Experience in social work, human services, or working with youth
Experience with at-risk youth preferred
Strong communication and organizational skills
Ability to build trust and maintain professional boundaries
Compassionate, dependable, and team-oriented
Ability to handle sensitive situations with professionalism
