Site: Martha's Vineyard Hospital, Inc. Mass General Brigham (MGH) is a not-for-profit health system. This role supports patient care by providing transportation and social resource assistance.

Job Summary
Assists patients to arrange transportation to/from the hospital for ongoing medical treatment, locate affordable accommodations and support groups, and coordinate referrals to community social resources.

Essential Functions

Research community social resources and maintain files of brochures, information packets, and applications.

Collaborate with social work staff and community agencies to obtain needed information.

Assist in planning and implementing in-service presentations about community resources.

Assess patient/family understanding of available resources.

Educate patients/families about appropriate resources and how to access them.

Make referrals to and disseminate information about a range of related social resources.

Qualifications

High School Diploma or equivalent required; Associate’s Degree in a related field preferred.

2–3 years of related experience preferred; experience in lieu of a degree not acceptable.

Familiarity with medical terminology and community services/resources.

Strong assessment and crisis intervention skills.

Excellent interpersonal, collaboration, customer service, advocacy, written, and verbal communication skills.

Physical Requirements
Standing, walking, and sitting frequently; lifting up to 35 lbs (with assisted device); carrying 20–35 lbs; other tasks as required.

Work Location & Hours

Location: One Hospital Road, Martha's Vineyard Hospital, Inc.

Weekly Hours: 37.5 regular day shift.

Compensation
Hourly rate $17.71 – $25.28; additional benefits include comprehensive benefits, career advancement opportunities, bonuses, and recognition programs.
