Hours
Self‑scheduled during the week. Monday through Friday. No weekends.

Service Areas
Howell, Oregon; Shannon, Texas; Wright, Douglas, and Ozark counties.

Highlights

PRN to Part‑Time (flexible scheduling)

$30.00–32.00 per hour

Responsibilities and Duties

Medication set‑ups and reviews

Skin assessments and general health evaluations

Client education and documentation

Enable patients to stay independent while they remain in their own homes

Perform routine supervisory visits

Follow up for unskilled services

Maintain a high degree of confidentiality at all times due to access to sensitive information

Maintain regular, predictable, consistent attendance and be flexible to meet the needs of the agency

Follow all Medicaid and HIPAA regulations and requirements

Abide by all regulations, policies, procedures and standards

Perform other duties as assigned

Qualifications

Graduation from an accredited school of nursing and current LPN licensure in the state of practice

New Nurses welcome, will train

Ability to make home visits and provide nursing care in the home environment

No weight restrictions

Home Care experience preferred but not mandatory

Must have reliable transportation, current driver’s license and appropriate automobile insurance, or ability to take public transportation

Ability to complete necessary documentation appropriately, legibly, and timely

Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
