to work with our Home Health team to deliver individualized care to patients in their homes throughout the
Naples, FL area
(Collier county).

Locations:
Home health visits in
Collier county
(including
Naples, Bonita Springs, Immokalee, Ave Maria , and surrounding areas)
Schedule : PRN role; 10+ home health visits per week. Required weekend/holiday on-call rotations.
What you'll Do
Perform comprehensive patient assessments, including OASIS assessments at required intervals
Develop, initiate, and revise individualized plans of care as patient needs evolve
Deliver skilled nursing services and treatments as outlined in the plan of care
Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need
Graduate of an approved nursing program with an active Florida RN license
Prefers BSN or MSN degree
Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred
Prefers experience using Homecare Homebase (HCHB) EMR
Strong assessment, critical thinking, and patient education skills
Willingness to participate in weekend and holiday on-call rotations
