RN Hospice Case Manager

Trustbridge Hospice, a proud member of Empath Health, is seeking a caring and motivated Hospice Registered Nurse (RN) Case Manager to join our dedicated hospice team in the northern Broward County area. In this vital role, you will provide exceptional, patient-centered care while guiding families through one of life’s most meaningful journeys on the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement.

What you’ll Do
Support and carry out the organization’s mission, vision, and values consistently.
Coordinate care for patients/families under the direction of the primary physician and registered nurse.
Identify patient/family needs.
Contribute and follow an individualized plan of care as part of the interdisciplinary group (IDG).
Utilize monitoring, intervention and teaching skills to maximize patient comfort and enhance quality of life for the patient/family unit.
Prioritize the comfort and care of patients and families.
Provide a high level of personalized, sincere, and engaging service to patients and families at all times.
Ensure complete satisfaction of patients and families, promptly and effectively addressing all concerns.
Maintain utmost respect for confidentiality in patient/family care.
What you’ll Need
Registered Nurse currently licensed in the State of Florida.
BSN preferred.
Hospice certification preferred.
Current Basic Life Support Certification.
Understanding and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs.
Basic computer skills and EMR software experience a plus.
Adequate and reliable means of transportation with insurance and a valid Florida driver’s license.
