Clinical Navigator
Location: Post Falls, ID. Facility: Kootenai Heart Clinics. Department: Kootenai Heart Clinics - Post Falls.
Assists patients in understanding their diagnosis, treatment options, and the resources available.
Responsibilities
Serves as an essential link between patients and all other medical staff
Facilitates appointments for consults and support services within established service standards, including social workers, dietitians, and physicians
Coordinates multidisciplinary planning conferences, develops concise patient summaries for use by the care team and documents recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines
Develops patient education programs and tools
Follows patients throughout the care continuum, including inpatient admissions and collaborates with inpatient care management resources
Adheres to established productivity benchmarks; identifies opportunities to reduce the cost of care
Responsible for outreach efforts to establish and maintain positive working relationships with medical staff
Performs other related duties as assigned
Familiar with standard concepts, practices, and procedures within the field
Relies on experience and judgment to plan and accomplish goals
Regular and predictable attendance is an essential job function
Competent to meet age specific needs of the unit assigned
Requirements and Minimum Qualifications
BSN required; MSN preferred
Graduate of a nationally accredited nursing program
Licensed as a RN by the Idaho Board of Nursing OR licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the "multi-state privilege to practice" compact with Idaho
Minimum of 3 years of relevant nursing experience required
Certification within specialty area required within one year of meeting qualifications
Must complete KH BLS during orientation period
Working Conditions
Must be able to maintain a standing position
