Responsibilities
Conduct psychosocial assessments and identify barriers to care for adult and geriatric patients in acute/short-term settings.
Lead discharge planning and care coordination to ensure smooth transitions from inpatient care to home or alternate placements.
Collaborate with interdisciplinary teams (PT, Interpretation Services, Social Services, Pharmacy, Transportation) to align patient care plans.
Perform case management across Med-Surg units, long-length of stay observations (LLOS), and the Emergency Department as needed.
Document assessments, plans, progress notes, and discharge summaries in the EPIC system.
Provide patient and family education, resource linkage, and community support referrals.
Participate in ongoing care coordination and crisis intervention as required.
Maintain compliance with hospital policies, licensing requirements, and patient confidentiality standards.
Requirements
Licensure: Active Washington state license in social work required; LMSW or LCSW accepted; WA license in-hand by start date.
Certifications: BLS (Basic Life Support).
Education: Master’s degree in Social Work (MSW) required.
Experience: Minimum 1 year of acute care experience; discharge planning/care coordination experience required; strong preference for case management across Med-Surg, LLOS, ED.
Clinical skills: Expertise in discharge planning, care coordination, interdisciplinary collaboration; experience in adult/geriatric social work within acute care settings.
Charting: EPIC experience required.
Priority notes: Locals not accepted; must be comfortable floating to multiple hospital areas; ability to work in a fast-paced Level II trauma center environment.
Additional: COVID-19 vaccination not required.
