Job Duties:
Serves as the first point of contact for internal and external customers regarding home health referrals
Receives, reviews, and documents patient referrals to ensure eligibility and compliance with coverage criteria
Verifies insurance coverage, Medicare eligibility, and obtains payer authorizations as needed
Enter referrals into the EMR system accurately and in a timely manner
Coordinates patient evaluations, admissions, and assigns field staff for initial assessments when directed
Communicates with Business Development and leadership regarding pending admissions and patients not meeting criteria
Maintains daily referral logs, admission reports, ROC assignments, and non‑admit documentation
Ensures HIPAA compliance and follows all company policies, procedures, and regulatory guidelines
Supports company core values and performs additional duties as assigned
Requirements:
Current Licensed Practical Nurse (LPN) in the State of Louisiana preferred but not required
Minimum of 1-2 years as a medical office experience
Computer skills and data entry experience
Goal-oriented and excellent communication skills
Good command of MS Office
Medical, home health or hospice office experience required
