FTE: 1.00 Full Time Shift: Days
Position Summary: Responsible for Patient Centered Medical Home (PCMH) and Patient Centered Specialty Practice (PCSP) patient outreach duties requiring medical record research and patient contact prior to patient visits. Perform a variety of front and back office clerical duties in support of assigned area to include but not limited to collecting patient data, obtaining patient medical history; performing treatment procedures, preparing instruments for sterilization; maintaining a sanitary work area; coordinating and scheduling appointments and verifying medical coverage. May perform a variety of clerical duties to include basic cashiering duties. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult and geriatric age groups.
Detailed responsibilities:
Adhere to and promote the core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice as applicable
Enter various data into computer and forms; verify data, make corrections and ensure accuracy; schedule patient appointments; answer and direct phone calls as requested; take and relay messages to providers
Review and ensure patient billing is accurate and in compliance with financial policies, procedures and contractual requirements
Maintain a sanitary work area to include patient care rooms, equipment and instruments; report equipment problems; order and stock supplies; maintain appropriate PAR levels
Provide and ensure quality service and customer satisfaction
Manage CERNER pools including medication refills, lab tracking, external specialty appointment tracking
Manage multiple registry lists including calling and scheduling patient for missed
eeded services/appointments
Support providers in a variety of clerical duties, such as preparing various patient forms for provider review and authorization, such as disability/FMLA/Prior Authorization, etc. as necessary
Follow patient safety-related policies, procedures and protocols
Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes
Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk
Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner
Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may 'not seem right'
Qualifications
Education
Essential: High School or GED Equivalent
Nonessential: Associate Degree, Course
Education specialization: Nonessential: Related Discipline, Medical Terminology Course w/in Six Months of Position
Experience
Essential: No minimum experience required
Nonessential: Bilingual English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo
Credentials
Essential: CPR for Healthcare/BLS Prov or Prof Rescuers within 30 days
Physical Conditions
Light Work: Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Physical demand requirements are in excess of those for Sedentary Work. May require walking or standing to a significant degree or requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or may require working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of materials is negligible.
