
CNA-Home Health Aide - Home Care Oak Creek

As part of our interdisciplinary Home Care team, our CNAs (Home Health Aides) provide patients with care in the comfort of their own home and/or similar setting. We serve patients throughout Milwaukee County.
As a Home Health Aide in Home Care you will provide direct assistance with activities of personal care, maintenance of patient health, Agency facilitation of treatment for patients, and contribution to a healthy environment for patients. These activities are provided under the direction and supervision of a Registered Nurse or Therapist, in accordance with the physicians' orders.
Responsibilities
Support patient care by reviewing the care plan in the home prior to providing care. Notify Case Manager with issues or concerns regarding the care plan and when care needs revision.
Provide assistance with daily living under the supervision of the Case Manager, e.g., bathing, dressing, transfers, range of motion, toileting.
Explain to the patient the care being provided and involve the patient in care.
Consistently complete mobile care upon arrival and departure from the patient's home per Agency policy. Complete mileage logs timely and accurately.
Work cooperatively with patients, care providers, and family members, as well as with appropriate Agency staff by listening and responding to patient needs, engaging in appropriate communication, and reporting pertinent patient information to Case Manager in a timely manner.
Regularly complete home maintenance per the plan of care, e.g., house cleaning, washing dishes, laundry, meal preparation, and transportation.

Qualifications
Wisconsin CNA certification that allows you to work in federally certified Home Health agencies and hospices.
Prior experience working with the public is required.
Current and valid WI driver's license plus a reliable, insured vehicle.
Job type: Full‑time, Monday‑Friday, first shift with weekend rotation and holiday rotation. Hourly, non‑exempt plus a $2,000 sign‑on bonus. Location: Home Care Oak Creek.
