Qualifications

Licensed Occupational Therapist

Key Responsibilities

Effectively screen/evaluate patients with cognitive and/or ADL disorders and develop appropriate plans of care following all regulatory and clinical practice standards.

Utilize standardized assessments and evidence‑based practice to support clinical interventions.

Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or ADL disorders per regulatory and clinical practice requirements.

Administer various procedures as part of the rehabilitation plan including, but not limited to, ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices.

Assess patient progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.

Evaluate treatment outcomes for effectiveness; modify services in relation to patient evaluation and refer when necessary.

Provide consultation and counseling to patients, families, caregivers, and other service providers related to the physical disorders.

Communicate effectively with supervisors and other health team members regarding patient progress, barriers, and treatment plans.

Provide comprehensive discharge summaries of services provided per regulatory and clinical requirements.
