Summary
The VA Central Iowa Healthcare System is recruiting for (1) full-time Advanced Practice Nurse (APN)- Hospice/Palliative Care. The Advanced Practice Nurse (APN) III Certified Nurse Practitioner (NP) practices in Rehabilitation and Extended Care (REC) Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice - consultation - and management.

The Hospice and Palliative Care Nurse Practitioner functions in a clinical role to provide Veteran care and promote patient-centered interdisciplinary approaches to those individuals referred to Hospice and Palliative Care services at VACIHCS in accordance with standards of Veterans Health Administration (VHA) and other accrediting bodies to include the following: These duties are performed on a routine and repetitive basis based upon the training and experience of the incumbent Hospice and Palliative Care Nurse Practitioner (NP)
Completes hospice and palliative care consultations in the inpatient and outpatient settings including components of detailed history and physical exam - review of diagnostics evaluation and management of symptoms and functional status - care coordination/recommendations - education and counseling in reference to goals of care and advanced care planning - spiritual and psychosocial support - prognostication - community support referrals and scheduled follow up visits
Facilitates collaboration of the interdisciplinary palliative care staff members locate throughout the medical center - promoting a team approach for the coordination of Home Based Primary Care (HBPC) services with a focus on providing seamless care - continuity along the continuum of care for individuals with advanced chronic and/or terminal illness
Manages ongoing treatment of follow-up patients - consulting with the Hospice and Palliative Care physician as appropriate based upon the NP's experience and delegation of duties
Maintains active interface with the VISN 23 Home Based Primary Care (HBPC) teams and leadership staff through participation in routine scheduled business calls - membership on VISN level task forces and VISN-facility program progress calls
Coordinates care for hospice and palliative care patients - ordering appropriate diagnostic tests - performs patient education regarding ongoing care of hospice and palliative care needs - prescribes medications and products as appropriate
