Duties

The Advanced Practice Nurse (APN) II Certified Nurse Practitioner (NP) practices in Primary Care Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission

The NP provides advanced clinical practice - consultation - and management

The NP practices to the full extent of their education - training - and certification

Core functions are outlined in the clinical privileges that correspond with this position

The NP has prescriptive authority for a panel of patients or program as indicated by their position

The Nurse Practitioner (NP) is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients

The Nurse Practitioner is a professional - independent practitioner with advanced education and appropriate experience who provides evidence-based care for a specific population of patients

The NP is an expert clinician who uses advanced assessment - planning and evaluating competencies - as well as approved standardized procedures - to provide family centered - preventive and illness management healthcare incorporating the patient's age - gender - culture and learning abilities in the treatment plan

The NP works collaboratively with many disciplines to provide care - including physicians - pharmacists - registered nurses - licensed vocational nurses - dietitians - social workers - clerical and administrative staff - community agencies and others as appropriate

The NP executes position responsibilities that demonstrate leadership - experience - and creative approaches to management of complex patient care

Major duties and responsibilities of the position include but are not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice Develops and implements an individualized plan to achieve optimal population health and disease management Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members Orders and interprets diagnostic studies Prescribes pharmacologic and non-pharmacologic interventions
