
Nurse Practitioner - Geriatrics

Summary
This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.
Duties
The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Primary Care Service on the Geriatrics team to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
The NP provides advanced clinical practice - consultation - and management
The NP practices to the full extent of their education - training - and certification
Core functions are outlined in the clinical privileges that correspond with this position
The NP has prescriptive authority for a panel of patients or program as indicated by their position
The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients
Major duties and responsibilities of the position: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
Develops and implements an individualized plan to achieve optimal population health and disease management
Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
Orders and interprets labs - tests - and diagnostic studies
Prescribes pharmacologic and non-pharmacologic interventions
Performs age-appropriate screenings and preventive health care measures for geriatric patients
Initiates referrals and consults to specialty services
Identifies psychosocial needs and coordinates appropriate referrals to social work - pharmacy - psychology - dietetics - and other disciplines
Provides education to patients and their families
Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

All VA NPs must have: National board certification in their NP role/population focus
An active - unrestricted RN licensure
