Job Summary
Pine Hill Rehabilitation and Wellness Center, a leading provider of skilled nursing, therapy, and rehabilitation services, is seeking a highly skilled and compassionate MDS RN to join our team. As a member of our interdisciplinary team, you will play a vital role in enhancing the lives of our patients and residents by ensuring they receive the highest level of care and personalized attention.
Responsibilities
Conduct comprehensive assessments of patients/residents using the Minimum Data Set (MDS) to identify their physical, emotional, and social needs
Develop and implement individualized care plans to address identified needs and achieve optimal outcomes
Collaborate with the interdisciplinary team to ensure patient/resident‑centered care and coordination of services
Implement and monitor treatment plans in collaboration with the care team
Participate in resident care conferences and MDS re‑assessments to ensure continuity of care
Maintain accurate and thorough records of patient/resident care and progress
Demonstrate a commitment to quality care and patient satisfaction
Uphold the highest standards of professionalism, compassion, and respect in all interactions with patients, families, and colleagues
Requirements
Current RN licensure in the State of Alabama
Minimum of 0 years of experience in a long‑term care or rehabilitation setting (new graduates welcome to apply)
Strong assessment, communication, and interpersonal skills
Ability to work effectively in a fast‑paced, dynamic environment
Familiarity with MDS and care planning processes
