Job Description
Care Coordinators coordinate in‑home and community‑based services for the elderly and people living with all types of disabilities.

Duties

Working Mon‑Fri 8a-5p. Please note the position will occasionally require the manager to work a slightly different schedule to accommodate client needs. This may include working direct care shifts, some evenings/ weekends.

Performing on‑call duties at least once every 8 weeks. The duties last for a duration of two consecutive weeks. These duties include answering and managing emergency phone calls from 5p-8a M‑F and all day Sat and Sun. These duties may also include working direct care shifts.

Maintaining and growing a caseload in Southern Arizona

Scheduling shifts for clients and direct care staff

Orienting and training direct care staff with clients

Monitoring direct care staff performance

Holding staff meetings as needed and attending weekly program department meetings

Conducting intake of new clients

Meeting with clients/families minimally every quarter

Attending meetings with other agencies involved in a client's care

Ensuring that all paperwork in client files is updated regularly

Documenting all significant conversations and/or correspondences with staff and clients

Maintaining client and agency confidentiality at all times

Qualifications

18 years of age or older

Ability to obtain a current CPR/FA certification

Ability to obtain fingerprint clearance

Ability to clear a DES background check

Ability to obtain a current T.B. clearance card

The ability to work in a team environment

The ability to handle difficult situations with clients/families

The ability to work a flexible schedule

The ability to learn and operate UCPSA program software

The ability to work independently

The ability to lift and transfer clients

Own/have regular access to and be able to operate a motor vehicle for the purpose of visiting and transporting clients, attending meetings outside of the office, etc. Proof of current registration and insurance of the vehicle must be kept on file with UCPSA. This position will require travel to Rio Rico, Nogales, Amado, etc.

At least one year of direct care experience, personally or professionally, a degree in a related field, and/or case management experience
