How You Will Contribute
The Admissions Coordinator plays a critical role in facilitating timely and accurate hospice admissions by coordinating referrals, patient records, physician orders, and patient/family appointments. This position serves as a key liaison among Community Liaisons, referral sources, patients, families, and our clinical team.
This is a hybrid role.
How You Will Make An Impact
Admissions & Referral Coordination
Receive and process referrals from Community Liaisons, physicians, patients, families, and community partners.
Coordinate requests for patient records.
Coordinate and schedule patient and family appointments for informed consent and RN assessment visits.
Verify and upload patient records to EMR.
Ensure timely notification of new referrals, admissions, and non-admissions to the assigned interdisciplinary team (IDT) and ancillary providers.
Coordinate supply and DME provision to meet patient needs.
Follow up on patients who have not been admitted and/or have been discharged.
Documentation
Maintain and organize patient charts, ensuring all documentation is complete, signed, and dated.
Transfer completed admission charts to the Team Coordinator in a timely manner.
Patient & Family Support
Explain hospice services, benefits, and eligibility to patients, family members, and referral partners, as needed.
Serve as a compassionate and knowledgeable point of contact during the admission process.
Team Communication & Collaboration
Inform appropriate team members of scheduled admission appointments and updates.
Communicate identified needs, issues, and potential solutions to the supervisor.
Attend and participate in staff meetings, educational programs, and community events, as requested.
The Experience And Skills You’ll Bring
Active, unrestricted Registered Nurse License.
Be available at all times during agency operating hours or as needed.
The ability to make sound professional clinical judgment.
Excellent verbal and written communication skills.
Proficiency with EMR and data entry systems.
Hospice experience preferred.
