LPN Unit Coordinator
to support daily clinical operations and provide leadership on the unit. This role is ideal for an LPN who enjoys coordinating care, supporting staff, and ensuring high-quality resident outcomes.
Key Responsibilities:
Coordinate unit workflows and resident care
Support nursing staff and ensure regulatory compliance
Communicate with interdisciplinary teams and leadership
Assist with documentation and clinical follow-up
Qualifications:
Active LPN license
Prior SNF/LTC or charge experience preferred
Strong organization and communication skills
