If interested in this RN Field Case Manager position APPLY NOW for IMMEDIATE consideration Brittney Blackman | bblackman@alinestaffing.com | text 5867107970 RN FIELD CASE MANAGER | DETAILS AND COMPENSATION: Location: Shiloh IL 62269 – 100% on-site, home health settings Covering : O'Fallon / Shiloh, IL Area (Serving St. Clair, Madison, Clinton, Randolph, Bond, Monroe, and Washington Counties) Payrate: $56.14/hr + $0.49/hr Required Availability: Full-Time | Monday – Friday, 8:00 AM – 5:00 PM Monthly on-call rotation RN FIELD CASE MANAGER | SUMMARY AND HIGHLIGHTS: The RN Field Case Manager will act as a patient advocate, ensuring high-quality clinical outcomes and seamless care transitions within the home setting. This role is responsible for performing comprehensive assessments of high-risk members and collaborating with an interdisciplinary team to implement individualized care plans. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates RN FIELD CASE MANAGER | RESPONSIBILITIES: Assessment & Care Planning: Perform detailed clinical assessments and collaborate with primary care providers to develop and implement comprehensive care plans. Care Coordination: Partner with physicians, social workers, and discharge planners to manage patient transitions and engage specialty resources for optimal resolution. Clinical Intervention: Identify opportunities for health promotion and intervene promptly to minimize adverse patient occurrences and poor outcomes. Documentation: Maintain precise records of clinical, functional, and fiscal outcomes during the management process, including OASIS documentation. Patient Advocacy: Ensure compliance with treatment plans by coordinating non-medical supports (such as housing and transportation) while protecting patient privacy and confidentiality. RN FIELD CASE MANAGER | REQUIREMENTS: Education: Associate’s or Bachelor’s Degree in Nursing (BSN preferred). Attendance: Commitment to mandatory attendance during the first 90 days. Licensure: Current, unrestricted Illinois Registered Nurse (RN) License. Certification: Case Management certification is required. Ability to analyze complex medical information and make rational, sound clinical decisions. Preferred Qualifications Home Health Expertise: Strong history in home health care including admissions, recertifications, and resumptions of care. Technical Skills: Experience with Infusion therapy, labs, wound care, Wound Vacs, ostomies, and tube feeds.
