Payrate: $56.14/hr + $0.46 per mile. Required Availability: Full-Time | Monday – Friday, 8:00 AM – 5:00 PM Productivity Benchmark: 30 points weekly. (Weekend on-call rotation required) HOME HEALTH RN CASE MANAGER | SUMMARY AND HIGHLIGHTS: The Home Health RN Case Manager will focus on performing clinical assessments, developing individualized treatment plans, and managing transitions of care to ensure optimal patient outcomes. You will act as a vital link between physicians, social workers, and families to provide high-quality, cost-efficient care. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates HOME HEALTH RN CASE MANAGER | RESPONSIBILITIES: Case Management: Perform comprehensive assessments of high-risk members and collaborate with primary care providers to implement individualized care plans. Clinical Intervention: Provide skilled nursing care, including wound care, IV administration, drain care, and G-tube management. Care Coordination: Manage patient transitions between care levels and coordinate with specialty resources, discharge planners, and social workers. Advocacy & Support: Identify non-medical barriers to care (such as housing or transportation) and put supports in place to ensure treatment plan compliance. Outcome Tracking: Maintain detailed clinical and functional records to monitor fiscal and health outcomes during the management process. Preventative Care: Identify opportunities for health promotion and illness prevention to minimize adverse patient occurrences. HOME HEALTH RN CASE MANAGER | REQUIREMENTS: Education: Associate’s or Bachelor’s Degree in Nursing. Attendance: Commitment to mandatory attendance during the first 90 days. Licensure: Current, unrestricted RN license in Indiana (Compact license accepted). Case Management certification is highly preferred. Proficiency in wound care, IVs, and basic patient education. Experience with Homecare Homebase (HCHB) is a plus. Must be able to cover Orange, partial Floyd, and Washington counties (mileage reimbursement provided). Take the Next Step!
