Internal Medicine Physician

StaffMed Health Partners is recruiting a Post-Acute Care Primary Care Physician for a leading national physician services organization in a SNF setting in Ann Arbor, MI.

• Compensation: Competitive base salary commensurate with experience, plus productivity and quality incentives

• Provide primary-care attending coverage to a SNF resident panel (typically 75-125 patients)
• Manage longitudinal care for long-term residents and short-stay rehab patients
• Conduct admission H&Ps, monthly recerts, change-of-condition visits, and care planning
• Coordinate with consultants, families, and the IDT (NPs/PAs, nursing, therapy, social work)

• BC/BE in Family Medicine or Internal Medicine
• Comfort managing geriatric patients with multimorbidity, dementia, and end-of-life needs
• Prior long-term care, primary care, or hospitalist experience preferred
• Active state license (or eligible) and DEA where applicable
