Internal Medicine Physician

StaffMed Health Partners is recruiting a Post-Acute Care IM Physician for a leading national physician services organization in a SNF setting in Wilmington, DE.

Position Highlights:

• Provide internal-medicine attending coverage to a SNF resident panel (typically 75-125 patients)
• Manage admission H&Ps, monthly recertifications, and significant-change visits
• Diagnose and treat the medically complex post-acute population: CHF exacerbations, COPD, sepsis recovery, post-op management
• Lead the IDT alongside NPs/PAs, rehab therapy, nursing, and social work
• Document in PointClickCare or facility EMR per CMS PDPM standards

Qualifications:

• BC/BE in Internal Medicine
• Strong comfort with chronic disease management, anticoagulation, and palliative medicine
• Prior SNF / sub-acute / hospitalist experience strongly preferred
• Active state license (or eligible) and DEA where applicable
