Overview
Registered Nurse (RN) – Home Health. Weekly Hour Guarantee: 40 hours. Location: Lake Charles, LA. Coverage Areas: Lake Charles and surrounding areas (50-mile radius). Mileage Reimbursement: Yes (From first patient to last patient). Shift Schedule: Days, Monday–Friday, 8:00 AM–4:30 PM. Dress Code: Solid-color scrubs. On-Call/Weekend Requirements: 1 weekday per week and 1 weekend per month. Pay Rate: $47-$48/hr.
Responsibilities
Clinical Excellence: Deliver skilled nursing care in the home setting, ensuring safe, effective, and patient-specific interventions aligned with the established plan of care.
Patient Assessment: Conduct comprehensive assessments to evaluate patient needs, update care plans, and monitor progress.
Documentation: Complete accurate, timely, and compliant clinical documentation for all skilled services provided.
Education: Provide essential teaching to patients and caregivers regarding disease processes, medication management, and home-based care techniques.
Care Coordination: Collaborate with physicians and interdisciplinary team members to report changes in condition and ensure coordinated, holistic care.
Adaptability: Manage a full-time field schedule while remaining flexible to patient needs, travel requirements, and operational priorities.
Qualifications
Active Registered Nurse (RN) license (ADN or BSN)
Current BLS certification
Previous clinical experience (Home Health experience preferred)
Reliable transportation and valid driver’s license
