Overview
We are hiring QMHPs to work in various programs, as our company continues to expand. You will be a part of a team that goes out into the community, providing mental health support services to adults who live with serious mental illnesses.
Position
1099 QMHP
Responsibilities
Provide Community Stabilization; 24 hours a day, seven days a week, to provide for short-term assessment, crisis intervention, and care coordination to individuals experiencing a behavioral health crisis.
Deliver brief therapeutic and skill-building interventions, engage natural supports, and implement interventions to integrate natural supports in the de-escalation and stabilization of the crisis; coordinate follow-up services.
Advocate and network to provide linkages and referrals to appropriate community-based services; assist individuals and their natural supports in accessing other benefits or assistance programs for which they may be eligible.
Report all incidents to their immediate supervisor.
Maintain working knowledge of policies and procedures applicable to the position and administer the service in alignment with DMAS and DBHDS guidelines.
Complete intake documents, Individualized Service Plans, Daily Progress Notes (within 24 hours), Discharge Summaries, and any other administrative tasks; ensure all needed signatures are captured and documents are completed in a timely manner.
