We are currently seeking a
Care Coordinator
who will be responsible for providing support services to families with high-risk children, regarding behavioral or emotional problems, parenting issues, and life stressors by developing and implementing bio-psycho-social assessments and services plans.
Expected Schedule: Full-Time, 35 hours/week; Flexible
Provide outreach and engagement to client upon referral to the program by conducting an initial phone intake process with the family, collecting basic information and presenting information to team to determine eligibility
Complete an assessment of the family including both strengths and areas of need through home visits and BERS and Ohio scales to develop a care plan
Provide weekly face to face direct service hours to client including support, advocacy, referrals and assistance with the development of positive parenting, safety planning, basic life skills and parenting a child with special needs
Develop a list of interested parties identified by the family to participate in Child Specific Team meetings-run by the care coordinator-multiple times throughout involvement with the family and follow through with recommendations made at the Child Specific Team meetings
Maintain and build community relations, through education and advocacy and participating in external meetings such as the System of Care meeting
Ensure that the Wraparound process is family driven and that families experience ongoing participation in and ownership of their care plans
Participate in case reviews and supervision and attends all regularly scheduled team meetings and trainings
Maintain up-to-date case records on each client family, including weekly contact reports, service agreements, evaluations and termination summaries
Minimum of 40% travel may be required
Participates in all safety programs which may include assignment to an emergency response team

Requirements
Bachelors Degree with one to three years prior experience or training in social services or Associates Degree in relevant field with a minimum of three years experience or training in social services
Experience working with children and families with significant behavioral health needs.
A valid driver's license, state mandated levels of auto liability insurance, and reliable transportation
Must complete WRAP trainings
Organizational skills
Comfortability working both by oneself and within a team
