Nassau Rehabilitation and Nursing Center is dedicated to providing high-quality, resident-centered care in a supportive and collaborative environment. We are seeking a compassionate and skilled Occupational Therapist to join our interdisciplinary rehabilitation team.
Job Responsibilities
Assess patients’ physical, cognitive, and emotional abilities to identify areas for improvement
Develop and implement individualized treatment plans tailored to patient needs
Provide therapeutic activities and interventions to improve daily living and functional skills
Educate patients and families on coping strategies, adaptive techniques, and assistive devices
Collaborate with physicians, nurses, and other rehabilitation professionals to ensure comprehensive care
Monitor patient progress and modify treatment plans as needed
Maintain accurate documentation of evaluations, treatment plans, and progress notes
Advocate for patient needs across care settings
Assist patients with transitions back to daily activities
Conduct home or workplace assessments and recommend modifications when appropriate
Stay current with advancements and best practices in occupational therapy
Candidate Requirements
Bachelor’s or Master’s degree in Occupational Therapy from an accredited program
Current New York State Occupational Therapist license
Certification from the National Board for Certification in Occupational Therapy (NBCOT)
Minimum of 2 years of experience as an Occupational Therapist
Nursing home or skilled nursing experience preferred
Strong knowledge of human anatomy, physiology, and psychology
Proficiency with electronic health records (EHR), including Net Health and Sigma (preferred)
Excellent communication, problem-solving, and critical‑thinking skills
Ability to work independently and collaboratively within a multidisciplinary team
