Position Overview
Full-time, salary-based position. Pay Range: $90,000 - $100,000 with bonus opportunities. Schedule: Sunday - Thursday.

The Wellness Director oversees day‑to‑day health care operations, staff supervision, assessment of residents, development of individualized care plans, and communication with families and the care team.
Responsibilities
Manage staffing needs of the Assisted Living/Memory Care community, interviewing, hiring, training, and supervising key roles.
Supervise the health care of all residents and disseminate information to families, staff, physicians, and third‑party providers.
Perform assessments of residents admitted to hospitals or skilled nursing facilities and maintain contact with families and discharge planners.
Complete pre‑admission assessments and update records every six months or as needed.
Initiate resident negotiated service plans and provide instruction to residents and families.
Perform ongoing assessments of residents' physical and psycho‑social needs and coordinate with other departments to assure proactive quality care.
Order medications, oversee schedules, audit medication records twice weekly, and ensure proper documentation.
Perform on‑call duties for staffing and scheduling, act as MCM when unavailable, and cover emergencies to ensure medication assistance is provided.
Ensure infection control guidelines are met.
Maintain knowledge of Assisted Living regulations, medication administration regulations, and occurrence reporting.
Qualifications
Demonstration of Core Values: Love, Kindness, Honesty, Goodness, Fairness, Respect.
Current nursing license issued by the appropriate state, first‑aid certification (RN).
Minimum of one year of experience working with the elderly in assisted living, skilled nursing or hospital environment.
Continuing education as required by law and participation in in‑service trainings.
Knowledge of procedures for administering treatments and providing resident‑care services, universal precautions, assessment process, negotiated care plans, and compliance with Nurse Practice Act.
Ability to read and interpret safety rules, operating instructions, procurement manuals, and deliver effective communication in English. Basic typing skills and computer literacy.
Team members scheduled to work at least 24 hours per week are eligible for benefits; families may also be eligible.
