Job Objective

The Hospice RN ensures quality and safe delivery of Hospice nursing services. This role encompasses on-call remote phone triage and visits in the field, both scheduled and as necessary. On-call services are rendered during nontraditional work hours providing service during weekend, evening, and holiday hours.
Minimum Qualifications

Education: Graduate of an accredited school of nursing.
Experience: At least 2 years recent experience in the delivery of hospice nursing services.
Licensure: Valid license as a Registered Nurse in the State of Connecticut.
Essential Job Functions

Triage calls and speak with patients and families to meet needs, including medication adjustments, supply needs, and implementation of changes to the plan of care (POC).
Provide assessment of patient care needs when necessary, during nontraditional work hours.
Make home visits to meet acute care needs and assess and intervene for changes in patient status.
Implement the coordinated plan of care for adult and geriatric clients served by the hospice and palliative care teams.
Visits may occur during overnight hours, on weekends or holidays.
RN pronouncement at time of death including notification of funeral home, hospice team, attending MD, and additional family members as needed.
Assess patient and family needs, considering physical, emotional, and spiritual aspects and gathering data on social, economic, and cultural factors that may influence health and quality of life.
Perform initial assessment visits and Start of Care, including obtaining required documentation for services, medications, and DME, and developing the plan of care.
Evaluate and perform ongoing assessment and revise the plan of care with interdisciplinary collaboration as needs and conditions change.
Attend and participate in care conferences.
Perform other duties as assigned consistent with skills and training and the agency’s mission and goals.
Physical And Mental Requirements

Ability to sit or stand for long periods of time.
Ability to speak clearly and be understood.
Able to hear sounds within some background noise (e.g., answering the phone, intercom).
Visual acuity to see computer screens and read printed documents.
Ability to use company software and equipment to accomplish electronic tasks.
Ability to concentrate on fine details with interruptions.
Ability to attend to tasks for more than 60 minutes at a time.
Ability to understand and relate to the reasons behind related concepts.
Ability to remember multiple tasks over long periods.
Ability to perform gross body coordination such as walking, bending, and reaching.
Ability to perform tasks requiring hand-eye coordination such as keyboard skills, writing, or handling papers.
Environmental Conditions

Rare exposure to blood, body tissue, or fluids.
Possible exposure to high/low temperatures.
Salary: $68.00 - $165.00 per visit
